Invitation to present to the Implementation Science Special Interest Group covering the implementation and effects of an innovative GP-Physiotherapist Model of Care for COPD.Talk description: Chronic Obstructive Pulmonary Disease (COPD) is a major global health issue characterized by high morbidity, mortality, and healthcare costs. COPD is usually managed in primary care until exacerbation occurs. Despite the existence of evidence-based guidelines, such as GOLD and COPD-X guidelines, COPD remains underdiagnosed and there are high rates of misdiagnosis. Management of COPD has also been found to be suboptimal, with inconsistent implementation of key strategies like pulmonary rehabilitation. Physiotherapists commonly work with people with COPD through pulmonary rehabilitation programs and in the acute care setting, but at this time, are underutilised in primary care. The INTEGRATED study aimed to address some of these challenges and examined a novel model of care where physiotherapists were integrated into primary care teams to support GPs in the early diagnosis and management of COPD. The study utilised a mixed methods and implementation science approach, drawing on the Theoretical Domains Framework (TDF) and the Consolidated Framework for Implementation Research (CFIR) to comprehensively evaluate the model's impact and its acceptability to patients and clinicians. This approach combined both quantitative health outcomes with qualitative insights, such as barriers and facilitators, ensuring a thorough examination of how integrating physiotherapists into primary care teams can assist in COPD care.
