The research will focus on leverage informatics to optimise pharmacist-led medication management reviews by embedding an electronic portal within an existing clinical information system (BESTMED), with strong consumer and partner engagement. This will be achieved through identifying residents most at need of an pharmacist-led medication management review, and creating a portal called electronic medication management review (eMMR) portal to ensure a transparent and structured process that timely and supports in medication management communication.

This project therefore aims to: (i) develop, implement, and validate a pharmacist-led and informatics-supported eMMR portal to enable targeted reviews and track the outcome of medication reviews in RAC, (ii) measure the impact of the intervention on medication use (e.g., potentially inappropriate medication use) and resident-focused outcomes (e.g., unplanned hospitalisation, falls) and (iii) evaluate the cost-effectiveness of informatics-supported MMR relative to current practice.

The project directly addresses the desired objectives of the MRFF Stream 4 (generating evidence to support the safe and effective use of medicines in residential aged care facilities). Outcomes include improving the health and wellbeing of Australians by supporting safe and effective use of medicines; improving the quality of care provided to residents and reducing hospital readmissions due to medication complications; and providing evidence to support the safe and effective use of medicines in residential aged care.
