Abstract

Transnasal endoscopic vidian neurectomy is a key procedure performed for treatment of vasomotor rhinitis. Understanding the anatomy of the vidian canal is a key element to the surgery. Wide maxillary antrostomy and ligation of the sphenopalatine artery provide the initial steps to accessing the nerve. The pharyngeal nerves and the vidian nerve are then identified and divided. Application of bone wax prevents delayed reanastomosis of the neural fibers. Symptomatic dry eye as a result of the procedure typically resolves within 1 month.
