    Abstract

    Balloon kyphoplasty and vertebroplasty are clinically effective methods. However, single observations of BKP show failure of the method without disclosing its source. We believe that especially in severe osteoporosis and osteonecrosis failure of BKP occurs due to the "stress shielding" and the high stiffness of cement. Furthermore, it is important to obtain intra-operatively a complete height restauration with BKP since remaining kyphosis will increase the negative effect regarding the forces onto the adjacent vertebrae once a vertebral body has been filled with cement. Therefore, in case of older fractures one has to consider to rather prefer bone preserving kyphoplasty over BKP, perform vertebroplasty for older fractures and consider newer cements.
