System archetypes and the dynamics of rural telehealth: understanding archetypal patterns and emergent consequences in the Australian context

Abstract

INTRODUCTION
While telehealth has emerged as a promising solution for improving healthcare access for rural and underserved communities, evidence reveals several challenges in telehealth care delivery, including communication barriers and difficulties in establishing effective patient-provider relationships. The introduction of telehealth into rural areas represents a complex intervention implemented into a complex, dynamic environment characterised by a large number of interconnected actors, rapid technological evolution and changing regulations. This complexity creates conditions where unintended consequences may emerge, often manifesting gradually over extended periods. Despite this inherent complexity in telehealth implementation, there remains a dearth of research investigating the long-term unintended effects of telehealth implementation in rural contexts.
PROBLEM STATEMENT
Despite the recognised importance of complex systems approaches in rural health research, there remains a gap in applying system dynamics to understand how telehealth implementation in rural areas generates unexpected outcomes, particularly in the Australian context. This study addresses this critical gap by applying systems archetypes (Senge, 1990) to analyse these dynamics, aiming to identify crucial leverage points for policy interventions and provide insights for effective telehealth planning and implementation in rural settings.
APPROACH
This study employed a qualitative system dynamics approach to model telehealth implementation in rural Australia, focusing on the identification and analysis of system archetypes to uncover recurring patterns that lead to the emergence of unintended consequences. This study broadly followed the first two steps (problem articulation and formulation of a dynamic hypothesis) of the modelling process proposed by Sterman (2000) across four steps: a) articulating the problem via stakeholders interviews, b) Constructing Causal Loop Diagrams (CLDs) from interviews using causation coding (Saldaña, 2021), c) Identifying systems archetypes, and, d) validating and refining archetypes through workshops with experts and iterative refinement by the research team.
RESULTS
We identified five system archetypes. These archetypes represented two use cases of telehealth in rural areas: a) Provider-to-provider Emergency departments (EDs) telehealth, and b) Patient-to-provider telehealth; each with distinct archetypal patterns. The archetypes operate simultaneously and are interconnected within the complex system of telehealth implementation in rural Australia. The five archetypal patterns are: a) drivers and constraints of telehealth adoption depicted in a “Limits to Growth” archetype, b) allocation of resources to on-site and telehealth care depicted in a “Success to the Successful” archetype, c) telehealth’s influence on on-site staff hiring depicted in a “Shifting the Burden” archetype, d) telehealth and the digital divide in rural areas depicted in a “Fixes that Fail” archetype, and, e) increased demand and healthcare utilisation due to telehealth depicted in a “Tragedy of the Commons” archetype.
DISCUSSION
The identified system archetypes reveal complex interconnections between telehealth implementation and rural healthcare delivery that generate unintended consequences through multiple feedback mechanisms. These patterns suggest that while telehealth offers important benefits for rural healthcare access, its implementation could inadvertently reinforce existing healthcare disparities through resource diversion, workforce dynamics,
